Transitional Care Package

  • Assistance with transition from hospital or home to a new care setting

  • Emotional support and coaching for both the client and family during transitions

On-Going Care Package

  • A set number of hours of care management per month
  • Assistance with scheduling appointments, community services and other personalized services
  • Access to 24/7 emergency contact

Dementia Care Package

  • Dementia specific assessments and care planning
  • Behavioral management strategies
  • Support for family caregivers, including education and coping strategies

Family Education and Support Package

  • Education and training for family
  • Emotional support and coaching
